To register for ACTS once formal registration
day has passed:
1.
Print and complete all registration forms from the
Family Handbook and prepare checks for fees as follows:

Registration Checklist (page 98)
·
Registration Form (page 91)
·
Enrollment Contract (page 92) Sign and submit this ONLY
if you have read the policies to which you are agreeing to follow.
·
Release of Liability (pages 93 and 94) Keep one copy for
your file, send one copy with registration.
·
New Family Introduction Form (96)
·
Background check Information Form (97) – You must also
include a copy of photo ID when registering by mail.
·
Dues Check - $95 payable to ACTS

·
Check for background check fee, $8 payable to ACTS
*nonrefundable
·
Checks for materials fees, payable to each teacher for
each class for each student (see note 2).
2. On
a separate piece of paper, list the class/classes in which you wish to
enroll your child/children. If a class is full, you may still wait
list your child for that class. Students in the first or second spots
of a waiting list often get into the class, beyond that their chances
are less likely. You will need to submit a materials fee check for
each class for each student, payable to the individual teacher(s).
The teacher’s name and materials fee amount are included on the class
description page. Note on the memo line of each check the
student name and class period.
If you choose to wait list your child, please also include a
materials fee check for that class as well. Payments for wait listed
classes are not cashed until/unless the child moves off the wait list
and into the class. You will be notified if/when the student moves
into the class and given the opportunity to confirm that enrollment.
All materials fees are refundable
until August 8th. On that date, materials fee
for a dropped class will be forfeited.
3.
Mail everything to the Registrar. If anything (form or fee) is
missing, your registration will be
delayed until that item is submitted. Please
email the Registrar for mailing address.
4. I
will contact you by email once I receive your registration paperwork.
At that time, I’ll let you know if you student(s) can be enrolled or
would be wait-listed, and we will determine final schedules and
confirm registration/enrollment.
